What Case Management Offers You
What is Case Management?

Case managers assist individuals determined eligible for services from the Missouri Division of Developmental Disabilities in accessing comprehensive medical, social, educational and other specialized services. Case managers are professionals who are trained in the field of mental health and/or closely related fields. Each individual who applies for services from the Division of DD is assigned a case manager who assists the person and his or her family to identify, locate, coordinate and monitor services that meet the individual’s distinct needs.

Eligibility Requirements

To be eligible for most services, an individual must have a disability.

Which is attributable to:

**Excluding only mental illness diagnoses.

With onset before the person attains the age of 22.

Is likely to continue indefinitely.

Which substantially limits two or more of the following major life activities:

Reside within Johnson County Missouri.

Referrals & Fees

Referrals to Johnson County Board of Services for individuals of any age may be made by the individual, school district officials, state agency representatives, physicians, or other interested parties. JCBS is a not for-profit agency funded through local, state and federal dollars billed through Medicaid. We do not have fees for case management services.
